Employee Accident Reporting


1. Purpose & Objectives

The Employee Accident Reporting Procedure ensures a structured and effective response to workplace injuries by:
Prioritizing the safety and well-being of the affected employee.
Providing clear guidelines for reporting and documenting workplace accidents.
Ensuring compliance with legal and regulatory requirements, including OSHA regulations.
Identifying risks and implementing preventive measures to reduce future incidents.

2. Prioritizing Employee Well-being

📌 In the unfortunate event of an employee injury or accident on company premises, the immediate well-being and safety of the affected employee is the top priority.

✅ If you witness or become aware of an accident, immediately notify your direct supervisor or the nearest on-duty manager.
Administer first aid if necessary.
✅ If the situation requires urgent medical attention, call emergency services without delay.

3. Thorough Reporting Process

📌 After ensuring the employee's immediate safety, the next step is to accurately document the incident.

✅ The injured employee, their direct supervisor, and any witnesses must complete an  Employee Accident Report.
✅ The report should include:

  • Date, time, and location of the incident.
  • Detailed description of how the accident occurred.
  • Nature of the injury sustained.
  • Names and contact details of any witnesses.

4. Regulatory Compliance

📌 In addition to the Employee Accident Report, the company is required to file  OSHA Form 301 for work-related injuries or illnesses.

✅  OSHA Form 301 : Provides a detailed account of the injury or illness, including the employee’s job title and contributing factors.
OSHA Form 300A: A summary of workplace injuries and illnesses, which must be completed annually and displayed in the workplace.

5. Ensuring Comprehensive Care

📌 If medical treatment beyond basic first aid is required, the injured employee should seek care from a company-approved healthcare provider.

✅ The direct supervisor is responsible for assisting the employee in arranging medical treatment and follow-up appointments.
✅ To support the employee’s recovery, regular communication between the supervisor and injured employee is encouraged.

6. Thorough Investigation

📌 The direct supervisor, relevant management, and safety personnel will conduct a detailed investigation into the incident.

Investigation Goals:

  • Identify the root cause of the accident.
  • Determine any contributing factors that led to the incident.
    ✅ Based on findings, corrective and preventive measures will be implemented to reduce future risks.

7. Regulatory Adherence

📌 Proper documentation and compliance with safety regulations are crucial.

✅ All forms, reports, and records related to the incident must be completed, filed, and submitted to HR in line with regulatory requirements.
✅ The company is committed to strict OSHA compliance and will ensure all safety regulations are met.
